昭和61年4月1日から平成3年3月31日生まれまでの方
（職場などで人間ドック等を受ける方、受ける予定の方を除く）

【内容】
基本的な健診項目
（身体計測、血圧、血中脂質検査、肝機能検査、代謝系検査、腎機能検査）
【健診期間】
令和7年7月1日（火）から10月31日（金）まで
【申込方法等】
◇ハガキ・・・必要事項（住所・氏名・生年月日・性別・電話番号）を記入の上、令和7年6月20日（金）までに郵送してください。
◇電子申請・・・URLから令和7年6月22日（日）までに申し込んでください。
